Superficial peels often use alpha-hydroxy acids or salicylic acid and stay within the epidermis; medium peels, typically based on trichloroacetic acid (TCA), reach the upper dermis; deep peels, usually phenol-based, go furthest and carry proportionally higher risk, so they belong strictly in medical settings.
Before treatment, the physician reviews your skin and history, and may ask you to prepare the skin for several weeks and avoid sun exposure. On the day, the skin is cleansed and degreased, then the solution is applied in sections; stinging and heat are common, and medium or deep peels may show a whitish “frosting” as the doctor times and neutralizes the reaction. Deep peels usually require pain control and monitoring. Afterward the skin feels tight and red, then flakes over days; superficial peels build gradual results over a series, while medium and deep peels show clearer change once healed.
The typical candidate is an adult bothered by dull or uneven tone, superficial pigment spots, acne marks, shallow acne scars, enlarged pores, or fine lines, who can accept some downtime. A worthwhile consultation should walk through your skin type and pigmentation risk, the depth and agent that fit your concern, how many sessions to expect, what each recovery period means for daily life, pre- and post-care requirements, and whether lasers, microneedling, or other tools would suit the goal better. Some situations argue against a peel or call for delay: pregnancy or breastfeeding; active infection, eczema, or open wounds in the treatment area; frequent cold-sore outbreaks without preventive antiviral cover; and a history of keloid or hypertrophic scarring. Anyone who has recently taken isotretinoin should tell the physician before a medium or deep peel so timing can be assessed. Expected reactions track closely with depth. After a superficial peel, redness, stinging, and fine flaking usually last from hours to a few days, and most people return to routine quickly. Deep peels have the longest recovery: dedicated wound care, marked swelling and crusting, and redness that can persist for weeks to months. At every depth, newly healed skin is unusually sun-sensitive, so strict sun protection is part of recovery.
Less common complications include post-inflammatory hyperpigmentation—more likely with darker skin tones or sun exposure during healing—hypopigmentation or visible demarcation lines (mostly after deep peels), infection, reactivation of cold sores, prolonged redness, and scarring. Phenol-based deep peels also carry systemic risk: the absorbed drug can affect heart rhythm, which is why they require monitoring in a properly equipped facility. For the same reason, medical-grade peels belong with licensed physicians in qualified medical institutions, not in ordinary beauty salons.
Contact the treating physician promptly if pain is severe or worsening, blistering or oozing goes beyond what you were told to expect, fever develops, the wound shows pus or spreading redness, healing is clearly delayed, or pigment changes keep deepening after recovery.
