In medical aesthetics, exosome therapy refers to using preparations of exosomes — nanoscale vesicles released by cells that carry signaling proteins, lipids, and RNA — most often applied to the skin to support recovery and calm redness after a resurfacing or microneedling procedure. It is an emerging and still evidence-limited category, and no exosome product is approved by the U.S. FDA for this use.
Exosomes are tiny vesicles that cells use to send signals to one another. Skincare and clinic products that contain or claim to contain them are typically used as a topical step during skin management — most commonly right after microneedling or laser treatment — with the goal of supporting barrier repair, easing post-procedure redness, and improving overall skin comfort. They are usually presented as an add-on to a procedure rather than a standalone treatment.
What exosome preparations can realistically do is still being studied, and marketing often runs ahead of the evidence. They are not a proven substitute for the medical treatment of a skin disease, and they differ from injectable options such as platelet-rich plasma or skin boosters, which are their own separate topics. Any promise of dramatic rejuvenation, guaranteed results, or a fixed number of days to “new skin” should be treated with caution.
The idea behind exosome therapy is that these vesicles carry molecular signals — proteins, lipids, and short strands of RNA — that may influence how surrounding skin cells behave, for example by dampening inflammation or supporting repair. Because vesicles are relatively large and healthy skin is built to keep such things out, products are generally applied when the barrier is temporarily open, such as immediately after microneedling or laser, or with a delivery device.
In practice, a session usually starts with a consultation and cleansing, followed by the microneedling or laser procedure that does most of the work; the exosome preparation is then spread over the treated skin and left to absorb, sometimes under a mask or cooling. The sensations you feel come mainly from the underlying procedure, not the serum. Calming of redness may be noticed over the following days, while any change in skin quality is gradual and, given the limited data, uncertain.
The people most often offered exosome preparations are those already having a microneedling or laser treatment who want extra support for recovery, or who have reactive, easily reddened skin. A worthwhile consultation should cover where the product comes from and how it is manufactured, whether it is sterile, whether it is part of a device-based procedure, its regulatory status, and how it compares with standard post-procedure care — which is often effective on its own.
It is not right for everyone. Anyone with an active skin infection, inflamed breakouts, or an open wound should hold off, and people who are pregnant, breastfeeding, or immunocompromised should be especially cautious with an unproven, human- or animal-derived biologic. Just as important, because these products are not standardized or approved for this use, someone who is uncomfortable with an unregulated preparation may reasonably prefer better-established options.
Because exosome preparations are usually paired with microneedling or laser, the visible recovery mostly reflects that base procedure: redness, mild swelling, pinpoint scabbing, dryness, or flaking that typically settles over several days to about a week. The topical product itself can also cause stinging, irritation, or an allergic reaction, especially on skin whose barrier has just been opened, so a patch of unexpected itching or rash is worth noting.
The more serious concerns are about safety and sourcing. The U.S. FDA has not approved any exosome product for aesthetic or other such uses, and has publicly warned that unapproved products — particularly when applied to broken skin — have been linked to infections and other adverse events. Because the source, manufacturing, sterility, and actual contents of these products often cannot be independently verified, there is a real risk of contamination or exposure to unknown biological material.
