The abdomen and buttocks are the most commonly treated areas, and some devices offer applicators for the thighs, arms, or calves. The primary target is muscle. Some devices, such as Emsculpt Neo, add radiofrequency heating intended to act on the fat layer at the same time, but that does not change the basic positioning: this treatment cannot replace regular exercise or weight management, and it does not treat obesity or loose skin. The boundary with fat-reduction procedures such as cryolipolysis is simple—those work on fat, while muscle sculpting works on the muscle underneath it.
During a session you usually lie down while the provider straps or positions an applicator over the target muscle group and gradually raises the intensity from a low starting point. The electromagnetic field stimulates the motor nerves directly, driving contractions far more frequent and forceful than voluntary exercise can produce. The sensation is often described as strong pulling, cramping, and rhythmic tapping; no anesthesia is needed, a single area commonly takes around half an hour, and you can ask for the intensity to be lowered at any time.

Case reports have also described rhabdomyolysis after high-intensity electromagnetic treatment: severe muscle pain, weakness, and dark, tea-colored urine that require prompt medical attention.
